摘要： 对先天性心脏病患儿出院后延续护理的模式、内容、形式、效果评价进行综述，以期为先天性心脏病患儿出院后延续护理的发展提供参考和借鉴。

Abstract: This review summarizes the mode, content, form, and effect evaluation of continuing care for children with Congenital Heart Disease after they are discharged from the hospital, hoping to provide reference and reference for the development of continuing care for children with Congenital Heart Disease after they are discharged from the hospital.
