Abstract: Objective: To develop and test the implementation intention questionnaire for stroke patients' rehabilitation exercise behaviors. Methods: Based on the concept of implementation intention, we developed the preliminary version through literature analysis, Delphi method and small sample pre-test. A convenience sample of 234 stroke patients were recruited to test the questionnaire. The questionnaire was evaluated using critical ratio method, correlation analysis, exploratory factor analysis, content validity, structural validity, internal consistency reliability, and test-retest reliability. Results: The questionnaire included 19 items. Two factors were extracted by using the exploratory factor analysis, with a cumulative contribution rate of 85.973%. The Cronbach's alpha was 0.980 for the total questionnaire and 0.982, 0.979 for each dimension, respectively. The test-retest reliability was 0.884 for the total questionnaire and 0.885, 0.856 for each dimension, respectively. Conclusion: The questionnaire has good reliability and validity. It can be used as a tool to evaluate the implementation intention of rehabilitation exercise behaviors for stroke patients.
