我国中西部6所三级甲等医院护士患者安全文化认知现状及影响因素分析

摘要: 目的:调查我国中西部6所三级甲等医院护士患者安全文化认知现状及影响因素。方法:采用方便抽样法,选取我国中西部3省6所三级甲等综合医院的3?996名护士为研究对象,应用美国卫生保健研究和质量机构组织的医院患者安全文化调查问卷进行调查。结果:护士患者安全文化调查问卷总体积极反应率为66.6%,各维度积极反应率为39.4%~86.2%;患者安全等级积极评级率为79.8%,一年内不良事件上报率为30.0%。科室监管者的期望和促进安全的行动、医院管理者对患者安全的支持、员工配置、组织学习和持续改进、差错的非惩罚性反应等可影响护士对安全的整体感知,可解释总变异的51.2%。结论:护士患者安全文化认知处于中等水平,受到多因素的影响。护理管理者应当重视差错的非惩罚性反应、员工配置、事件的报告频率领域的改进,客观认识患者安全文化对护士认知的影响,持续改进护理管理以促进患者安全。

Abstract: Objective: To investigate the nurses' perception of patient safety culture in central and western China and its influencing factors. Methods: A convenient sampling method was adopted to select 3996 nurses from 6 general hospitals in 3 provinces in central and western China. The survey was conducted by using the Hospital Patient Safety Culture Questionnaire developed by the US Healthcare Research and Quality Organization. Results: The overall positive rate of the Hospital Patient Safety Culture Questionnaire was 66.6%. The positive rate of each dimension was 39.4%-86.2%. The positive rate of patient safety grade was 79.8%, and the reporting rate of adverse events within one year was 30.0%. Supervisor/Manager Expectations and Actions Promoting Patient Safety, Management Support for Patient Safety, Staffing, Organizational Learning and Continuous Improvement, Non-punitive Response to Error and other factors could influence the nurses' overall perception of patient safety, which could explain 51.2% variation of nurses' overall perception of patient safety. Conclusion: The perception of nurses' patient safety culture is at a medium level, which is affected by many factors. Nursing managers should pay attention to non-punitive response to error, staffing configuration, improvement of incidents reporting frequency, objectively understanding of the influence of patient safety culture on nurses' perception, and continuous improvement of nursing management to promote patient safety.
