急诊科护士的工作疏离感和其对攻击及暴力管理态度的现状分析

摘要: 目的:调查急诊科护士的工作疏离感现状,分析其对医院暴力管理态度的影响。方法:使用便利抽样法,于2018年7-9月间选取7所浙江省三级甲等医院的252名急诊科护士作为研究对象。使用攻击及暴力管理态度量表(MAVAS)和护士工作疏离感量表进行问卷调查。结果:急诊科护士的MAVAS得分为(90.35±16.48)分,各维度得分从高到底依次为内部原因维度、医护与患者相互作用维度、外部原因维度和管理维度。工作疏离感得分为(45.12±7.23)分。多元回归分析结果显示,曾遭遇医院暴力、急诊科工作年限、工作疏离感和暴力相关培训可影响急诊科护士的攻击及暴力管理态度,可解释总变异的51.1%。结论:急诊科护理管理者应加强护士的暴力防控相关培训和演练,提高护士的防范和应对意识,采取个性化措施降低工作疏离感,保证暴力管理方法的有效执行。

Abstract: Objective: To explore the relationship between work alienation and the management of aggression and violence attitude of emergency nurses. Methods: Totally 252 emergency nurses from seven tertiary grade A hospitals were recruited by convenience sampling between July and September 2018. The Management of Aggression and Violence Attitude Scale (MAVAS) and the Nurses' Work Alienation Questionnaire were used in the investigation to collect relevant data. Results: The average score of MAVAS was 90.35±16.48 and the score of each dimension from high to low were internal reasons, interaction between medical workers and patients, external reasons and management dimension. The average score of work alienation was 45.12±7.23. Multiple regression analysis showed that experience of hospital violence, working time in emergency department, work alienation and training related to violence could influence the management of aggression and violence attitude of emergency nurses, which could explain 51.1% variation of MAVAS. Conclusion: Nursing managers in emergency department should strengthen the training and exercise of violence prevention and control, improve nurses' awareness, especially those who work in the emergency department for a short period of time, and take individualized measures to reduce the sense of work alienation, and ensure the effective implementation of violence management methods.
