Abstract: The information technology has been applied to transitional care more and more frequently abroad, but in our country transitional care still depends on the traditional family visits and telephone follow-up. This article reviewed the research progress of remote transitional care in patients with heart failure in the United States, summarized its inspiration for the development of remote transitional care in China, in order to provide basis for the implementation of remote transitional nursing for patients with heart failure, and thus promote the better development of transitional care.
