Abstract: Objective: To extract and summarize the relevant evidence on discharge preparation services for diabetic patients both domestically and internationally, in order to provide evidence-based basis for the implementation of hospital discharge preparation services for diabetic patients in China. Methods: We systematically searched the literature on hospital discharge preparation services for diabetic patients from domestic and foreign guideline websites, professional society websites, and databases, and evaluated the quality of the literature, and extracted evidence from the literature that met the quality standards. The search period spanned from the inception of the database to March 27, 2023. Results: Twelve studies were included, and a total of 28 pieces of evidence were synthesized, including the significance of discharge planning, process optimization, intervention objects, intervention principles, hospital services, pre-discharge connection and post-discharge follow-up. Conclusion: Medical staff should emphasize the key role of in-hospital health guidance, strengthen multi-link communication, and transform evidence into practical and feasible individualized diabetes discharge preparation service plans based on clinical situations.
