跌倒损伤入院老年人单次与多次跌倒特征及危险因素的比较研究

Comparison of characteristics and risk factors of single and multiple falls in the elderly admitted to the hospital with fall injuries

摘要: 目的:调查跌倒损伤入院老年人单次、多次跌倒特征,比较分析多次跌倒的危险因素,为预防及干预老年人跌倒提供参考依据。方法:采用横断面调查,收集跌倒后入院老年人的健康状况、身体结构及功能、活动和参与、环境及个体方面的资料,根据过去一年跌倒次数分为单次、多次跌倒,比较分析两组老年人特征及危险因素。结果:共纳入201位跌倒损伤入院老年人,其中43例老年人经历多次跌倒,占总人数的21.4%。多次跌倒老年人与单次跌倒老年人在尿失禁、疼痛情况、上肢肌力异常、助行器使用方面差异具有统计学意义(P<0.05)。结论:跌倒损伤入院老年人中,多次跌倒与疼痛情况、上肢肌力异常、助行器使用有关,护理人员可早期识别高危老年人,组织多学科团队进行干预,从而降低老年人一年内发生多次跌倒及损伤的风险。

Abstract: Objective: To investigate the characteristics of single and multiple falls in the elderly admitted to the hospital with fall injuries, compare and analyze the risk factors of multiple falls, and provide references for clinical prevention and intervention. Methods: A cross-sectional survey was conducted to collect data on the health status, physical structure and function, activities and participation, environment and individual information of the elderly admitted to the hospital after falls. According to the number of falls in the past year, the participants were divided into the single fall group and the multiple falls group, then the characteristics and risk factors of the two groups were compared and analyzed. Results: A total of 201 patients were enrolled in the study, of whom 43 experienced multiple falls, accounting for 21.4% of the total number. There was a statistically significant difference between the elderly with multiple falls and single fall in terms of pain, abnormal muscle strength in the upper limbs, and the use of walking aids (P<0.05). Conclusion: Among the elderly people who have been injured by falls, multiple falls were related to pain, abnormal muscle strength in the upper limbs, and the use of walking aids. Nurses can identify high-risk elderly at an early stage, and organize multi-disciplinary teams to intervene, thereby reducing the risk of multiple falls and fall injuries in a year.
