一站式快捷术前准备模式应用于关节置换择期手术患者的效果评价

The effect of one-stop rapid pre-operative preparation mode in patients with total joint arthroplasty

摘要: 目的:分析一站式快捷术前准备模式在关节置换择期手术患者中的应用效果。方法:采用回顾性队列研究,将2017年12月到2018年5月拟住院行关节置换手术患者作为对照组,2018年6-11月的患者作为干预组。对照组遵循传统的入院流程及术前准备模式,干预组实施一站式快捷术前准备模式,即通过开设加速康复外科院前联合门诊,应用术前检查前移系统,对关节置换择期手术患者进行医护小组协同化院前全身健康筛查与处理。比较两组患者平均住院时间、非计划出院率和住院满意度。结果:干预组患者比对照组的平均住院天数减少[0~8(5.68±0.86)天vs?0~10(6.83±1.24)分]、非计划出院率降低(4.9%?vs?15.6%)、住院满意度提高[(94.39±4.32)分vs(92.30±4.89)分],两组比较差异有统计学意义(P<0.05)。结论:一站式快捷术前准备模式可改善关节置换择期手术患者入院服务流程,降低非计划出院率,提高患者满意度,改善就医体验。

Abstract: Objectives: To explore the effect of one-stop rapid pre-operative preparation mode in patients undergoing total joint arthroplasty. Methods: Retrospective cohort study was conducted. Patients with joint replacement surgery from December 2017 to May 2018 were assigned as the control group, and patients from June to November 2018 as the experiment group. The control group followed the traditional admission procedure and preoperative preparation mode. The experiment group implemented a one-stop quick preoperative preparation mode, which included setting up a combined hospital enhanced-recovery surgical outpatient clinic, putting forward the pre-operative examination, carrying out pre-hospital health screening and treatment by medical and nursing team for patients undergoing total joint arthroplasty. The average hospital stay, unplanned discharge rate, hospitalization satisfaction of the two group were compared. Results: Patients in the experiment group had lower average hospital stays [0~8(5.68±0.86) d vs 0~10 (6.83±1.24) d], lower unplanned discharge rates (4.9% vs 15.6%), and improved hospitalization satisfaction [ (94.39±4.32) points vs (92.30±4.89) points] than the control group, the differences between the two groups were statistically significant (P<0.05). Conclusions: One-stop rapid pre-operative preparation mode can improve the hospitalization service flow, reduce the unplanned hospital discharge rate and improve satisfaction rate and medical experience of the patients.
