摘要： 本文阐述了产时会阴损伤发生的部位和原因，基于循证证据，总结国内外降低会阴损伤的干预方法，包括会阴按摩、产前盆底肌和瑜伽球锻炼、自由体位、延缓用力、硬膜外麻醉、限制性会阴切开、无保护会阴接产、对产妇持续的支持等。基于我国国情，提出改进产前助产保健服务、推进产程中自由体位开展、建立适合我国国情的分娩镇痛医疗服务体系、明确会阴切开评估指标、改进产时分娩服务模式等重点干预方向。

Abstract: This article described the positions and causes of perineal injury during delivery and summarized interventions to reduce perineal injuries based on evidence, including perineal massage, antenatal pelvic floor muscles and yoga ball exercise, free position, delayed pushing, epidural anesthesia, limited episiotomy, hands off technique, continuous support for maternal and so on. Based on our national conditions, some key interventional dimensions were proposed, such as improving prenatal health services, promoting free positon during delivery, establishing the labor analgesic medical service system suitable for Chinese conditions, clarifying indicators of episiotomy, and improving the delivery service system.
